Complete Pathological Response to Durvalumab in MSI-H Intrahepatic Cholangiocarcinoma Complicated by Drug-induced Sarcoid-like Reaction: A Case Report

We present a rare case of complete pathological response in a patient with microsatellite instability-high cholangiocarcinoma treated with durvalumab, complicated by a drug-induced sarcoid-like reaction. This report highlights the profound efficacy of immune checkpoint inhibitors in microsatellite instability-high tumors, even in rare malignancies such as cholangiocarcinoma, while also illustrating the potential for complex immune-related adverse events, such as sarcoid-like reaction.
