An Ethical Dilema Regarding Treatment Escalation in a Patient Lacking Capacity to Consent

A 74 year old female with a history of COPD and rapidly progressive Parkinson’s disease presented to the Emergency department with shortness of breath. She was diagnosed with infective exacerbation of COPD but despite optimal medical treatment, she deteriorated, becoming drowsy and confused with reduced GCS (Glasgow Coma Scale). Since she developed type 2 respiratory failure, the consultant believed non- invasive ventilation (NIV) was indicated.
